What Information I Collect

I collect personal information such as your name, contact details including phone number and email address, date of birth, gender, GP, payment information, Emergency contact details and relevant medical information necessary to provide counselling services.

I also keep brief session notes and general information around your reason for contacting my service.

 

Why do I collect this information and how do I get it

Your information is used to:

  • Provide counselling services

  • Maintain records

  • Arrange appointments

  • Meet legal obligations – safeguarding, insurance and tax  requirements

  • Supervision – (anonymised)

 

The personal information we receive is provided to me by you. The brief notes are made by me (Maria Pellow) and you have access to these at any point.

How your data is stored and retained

Records are stored securely in locked cabinets. I take reasonable steps to ensure your data is protected. For adults, records are kept for 7 years after therapy has ended. For Youn people under 18, records will be kept for 7 years after they turn 18.

This is disposed by a cross shredder and any online or computer data is removed.

Your data protection rights

Under data protection law, you have the rights including:

  • Access to your data

  • Rectification – You have the right to rectify personal information that you think in inaccurate or information you think is incomplete.

  • Erasure (where applicable) – You have the right for your personal information to be erased in certain circumstances

  • Restriction/object of processing – You have the right to restrict/object the processing of your personal information in certain circumstances

  • Data Portability – You have the right to ask that we transfer your personal information you gave us to another organisation, or to you.

You will not be charged for exercising your data protection rights. Once I receive your request, I will respond within one month.
